About the Canadian Medication Incident Reporting and Prevention System (CMIRPS)

ISMP Canada, the Canadian Institute for Health Information and
Health Canada
worked closely to develop the Canadian Medication Incident Reporting and Prevention System (CMIRPS). These
parties are working with the
Canadian Patient Safety Institute to coordinate CMIRPS with other patient
safety initiatives.
CMIRPS is a voluntary, confidential program that collects, analyzes, and distributes information on
medication incidents. The knowledge gained through analyzing reports submitted to CMIRPS is used to
make medication use safer.
Sharing information and learning from medication incidents helps to improve patient safety and quality
of care across Canada. Until recently, CMIRPS collected information mainly from individual healthcare
practitioners and hospitals. SafeMedicationUse.ca provides a convenient way for consumers to report medication
incidents and to get information about using medication safely.
